Unity, a not-for-profit healthcare provider in Northeast Wisconsin, has received a $25,000 grant from the K.C. Stock Foundation to expand its Supportive Care Management (SCM) program. The SCM program offers home-based palliative care for individuals with serious illnesses.
“We are deeply grateful to the K.C. Stock Foundation for recognizing the growing need for accessible palliative care,” said Unity Executive Director Alisa Gerke. “This generous grant ensures that more people throughout our region can receive the relief, dignity, and support they deserve during some of life’s most difficult moments.”
The SCM program delivers skilled nursing visits, 24/7 nurse consultation, emotional and spiritual support, and connects patients to essential resources. Unity states it is committed to providing these services regardless of a patient’s ability to pay.
In 2024, Unity provided care to 424 patients through SCM with over 5,100 care team visits. According to Unity, participation in the program leads to fewer emergency room visits or hospitalizations and helps reduce social isolation among patients.
With the new funding from the K.C. Stock Foundation, Unity expects to serve approximately 594 patients in 2025. The organization aims to help more individuals remain at home while receiving tailored medical and psychosocial support.
Unity describes itself as Northeast Wisconsin’s only locally owned advanced illness provider and collaborates with regional hospitals and other healthcare facilities. It was established as one of the first hospice programs in the United States and has introduced several firsts in palliative and hospice care within Wisconsin.
